I would like to add a pdf or word form that visitors to my site can fill out and submit (without using email). Is there a plugin that will allow this?

Contact Form 7 is the easiest plug-in and the most configurable one. It has many features, yet is easy enough to use. A major plus: it can handle multiple forms.
